Using the NTSB Aviation Accident Database (Links to an external site.), locate an aviation-related accident that involved a pressurization system failure from the last 5 years (an open-source military report is acceptable, but please do not include any “privileged” data). You could also use an incident of decompression sickness/bends related to a system failure (there are several examples in the U-2 and F-18 communities). Please avoid hypoxia events as we will cover that in detail later in the course.

Next, analyze and write a case analysis that discusses the factors which resulted in the pressurization failure and explain how this pressurization failure relates to flight physiology. Support your work with a reliable source(s).

Your analysis should include the following sections:

Summary of the case
Problem Statement What is the problem?
Significance of the Problem Why this is a problem?
Alternative Actions (2) How could it have been avoided?
Recommendation
