For this assignment, you will create a business survey. Review the information in your text about surveys.
You need to submit three items: a plan or overview, a cover letter to go with the survey, and a survey consisting of 6 to 8 items. Guidance for the cover letter and the survey is provided in the document shown lower on this page.
The plan is a brief summary (two paragraphs) of the following information:
1. Why is the survey needed? For example:
The business at my restaurant is declining on Tuesdays, and I need to know why and what would get the business back.
I am starting a web design business for chiropractors, and I need to know what they need and like.
My dry-cleaning company is considering offering a new pick-up and delivery service, and they need to know if their customers would use it and how much they would pay.
I want dog-owners to use my new mobile dog grooming service. I need to know how much to charge to get them to use it.
2. Who is the target of the survey? For example:
Chiropractors in south Orange County or their office managers.
Current and former patrons of my restaurant.
Current and former customers of my dry cleaning business.
Dog-owners in south Orange County.

Your survey development consists of (1) a plan, (2) a cover letter, and (3) the survey document itself.
Use the points in this document to develop your cover letter and survey. Your instructor will use these criteria to evaluate your survey.
Your survey should consist of 6 to 8 items that ask the respondents to rate something. Use a 5-point scale.
Three examples of items requiring rating:
Importance. How important is it for your shipping to be faster than your current service? (Extremely Important, Very Important, Important, Not Very Important, Not Important)
Likelihood. How likely would you be to use this service if it were priced at $25? (Extremely Likely, Very Likely, Likely, Not Very Likely, Would Not Use)
Satisfaction. How satisfied are you with the present selection of colors? (Extremely Satisfied, Very Satisfied, Satisfied, Not Very Satisfied, Not Satisfied at all)
