Convene your formative and summative committees and follow the specific guidelines on survey construction and development to develop a survey instrument. The instrument must contain an appropriate number of items to address your survey objectives. Include a 1-2 page synopsis with the (a) type of survey (i.e., web, mail, telephone, mixed-mode) you will utilize, along with the advantages and disadvantages of the type of survey you chose; (b) a brief description of the development of your instrument (make sure to discuss the use of the formative and summative committees); and (c) strategies to increase the response rate.
