primary and secondary data. I think therefore you need many more questions, certainly for the survey (around 20?).
For the Questionnaire/survey you need statements that do not lead with a choice of strength of agreement/disagreement. For example:
‘Stress impacts on the productivity and growth of the organisation’ Strongly Agree, Agree, Neither Agree or Disagree, Disagree, Strongly Disagree. Have about 20 statement/questions like this
that allow the respondents ti indicate their strength of preference. It will also need a covering letter to explain it.
