Performance appraisal forms mean a lot of things to a lot of different people and organizations. Design one appropriate for your own organization. Provide the complete form, as it would appear. Identify at least two theories of motivation that the form is intended to address and describe briefly how the form effectively addresses that theory. Then, provide the uses for whom and for which the form you have designed has been intended. Last give the frequency of the form and provide a description of the training that would be given along with the intended audience – multiple approaches are encouraged. Please remember to submit a bibliography as well.
Electronic submission using the course website is preferred. The length of the paper is to be between five and eight pages of “word processed” and double spaced text on 8 1/2″ x 11″ paper, not including the form itself.
