Healthy People 2020: Aggregate Health Disparities Report
Assignment Guidelines
Directions: This is an individual presentation. The grade is derived from the Power Point presentation content that addresses all of the criteria below.
Objectives:
Use the concepts of aggregate at risk and health disparities for assessing needs and planning care.
Determine the effects of social, political, economic and cultural systems on the health disparity of the at-risk aggregate.
Determine the health status of the at-risk aggregate by interpreting demographic, epidemiological, and statistical data.
Demonstrate an understanding of how and why the resources available to U.S. citizens vary among aggregates and how this could contribute to a disparity.
Analyze current research concerning the at-risk aggregate, including Healthy People 2020.
Formulate three prioritized outcomes with intervention strategies at each level of prevention (primary, secondary, tertiary) for the at-risk aggregate.
1. Select an at-risk aggregate and examine the aggregate in regard to the following areas:
Demographics, Epidemiological and Statistical data for the at-risk aggregate
Health status, including prevalent health problems of the at-risk aggregate
Social, political, economic (how and why economic factors vary), and cultural factors affecting the aggregate
Health beliefs and practices; use of alternative or complementary therapies
Healthy People 2020 (Links to an external site.) National Objectives related to the at-risk aggregate
CDC Sortable Stats (Links to an external site.) is an interactive data base with data on 31 Behavioral Risk Factors ) provides easy access to extensive public health data on a state/region and enables comparison with other states/regions and the nation.
2. The student will develop a prioritized plan of care for the aggregate.
Here are some tips to get you started on the Health Care of an Aggregate Assignment:
Specify if you are focusing on Prevention or the Management of a condition (e.g., prevention of diabetes in Latino teens between the ages 13-18 attending Seminole County Schools; versus management of diabetes in Latino teens between the age of 13- 18 attending Seminole County Schools)
Introduce the at-risk aggregate and the associated disparity. Include the demographics, epidemiological and statistical data. Pictures and graphics add to the presentation. Be sure to cite the sources of these materials and be aware of copyright issues!!
Proceed to the health status – prevalent health problem/disparity.
Delineate the social factors; political factors-include any key legislation at national level; economic factors ‘answer how and why’ resources vary; cultural factors -include the health beliefs and practices and use of alternative and complementary therapies in the cultural factors section. Add in specific Healthy People 2020 (Links to an external site.)objectives and supporting statistics related to the at-risk aggregate.
Identify the prioritized problem, need, risk list – you only need three; write these problems, needs, risks as outcomes for the aggregate. Develop the plan of care (intervention strategies) at each of the three levels of prevention – primary, secondary, tertiary. -Remember that you writing the plan in the role of the public health nurse.
Be sure to consider development stages of your target population. (e.g., teaching a group of 1st grades about preventing obesity will be quite different from teaching a group of 5th graders versus students in junior high)
3. VERY IMPORTANT!! By MARCH 1
When selecting your topic consider all of the following criteria (formula). Then send a course email to the instructor that includes all of the following criteria for your topic for approval of your topic.
a health problem/disparity (e.g, obesity, pregnancy, osteoporosis, diabetes, hypertension, alcohol abuse, dental caries, etc.) and your focus area that is prevention of a the condition (e.g., prevention of diabetes in AA males ages 30-45 years living in Volusia county) versus management of the condition (e.g., manamgent of diabetes in AA males ages 30-45 years living in Volusia county)
a specific ethnic/racial/religious group (e.g., Hispanic, Arab, Native American)
a specific age range (e.g, between 13-17 years of age; 35-50 years of age) (consider human development concepts)
particular gender (male, female)
a specific community/county (e.g., Rockledge high school, Volusia county senior center, Pilgrim Baptist Church, Flagler County etc.)
