Law : reinstantment form for a college

Law : reinstantment form for a college

case:

In Fall 2014, I joined the school   and I took the following courses
CHE   2005  3 credit      F
CHE   3015  3 credit      F     and I was missing the prerequisite for this course
CHE   3025  3 credit      F    and I was missing the prerequisite for this course
CHE   3990  6 credit      S
PETE 4990  3 credit      F
My overall GPA for this course was 0.0  and total credit number
After this semester they put me in probation.
In spring semester , I have tried to take the courses I failed but these courses are offered once a year only during Fall semester but I took the following instead:
CHEM 4515       3 credit   A-
ES       1060       3 Credit   C
POLS  1000       3 credit    B+
RELI    4500       3 credit    B
ES       4910       3 credit    D+
total number of credits is 15      semester GPA was 2.67

During summer I have taken the following courses

CHEM   4507    3 credit   Incomplete
MGT      3210   3 credit    F  , I was trying to drop it but I had an accounting hold in my school account and this prevented me from dropping the course at the deadline but I was able to get the approve from my academic advisor, the instructor , the head of the department and the dean of college to drop the course after the deadline at the end the registrar office denied it .

MOLB    3000   3 credit   C

Total credits is 9 and semester GPA is 1.0

After summer I got suspended  and this is was 6 days before fall started.

In the current time, I am trying to get help to write a petition and explaining my situation for them properly so they can give me a chance to retake the fall classes from 2014 and do a better job .Below are the questions they gave me answer in an Essay:

1- What have you accomplished of an academic nature that would indicate that you have corrected the conditions which contributed to your earlier academic suspension?
2- What has occurred in your life that has increased both your desire and ability to perform university-caliber work?
3- If reinstated, what commitments will you make to ensure that your academic performance will be acceptable?  (i.e., 2.0 GPA, study habits, living arrangements, employment arrangements, tutors, etc.)

Some things, I am going to do if the give the chance
CREATE A WEAKLY SCHEDULE TO STUDY EVEY DAY AND ORANIZE MY TIME.
PREPER FOR CLASSES AHEAD OF TIME.
ASK THE PROFESSOR FREDUITLY
GO TO THE TUORTING HOURS
HAVE A PRIVATE TOUTOR

Some information about me , I am 24 years old, international student from Spain and I am under a scholarship from my county. I come to the USA to seek better knowledge and I started studying English as a second language from the scratch  until I made it to the third year of college . Thus, losing this case will lead to losing the scholarship specially there is no enough time to transfer to a different school and also if I stay without taking classes my student visa will be determined.

write a strong supportive letter to give me the last chance to fix my past mistakes
