Type of incident (type of attack, the intended target), purpose (compromised confidentiality, integrity, availability) all or none or a combination.
How the incident happened or how the attack was conducted.
The vulnerabilities that were utilized to conduct the incident.
Countermeasures that could have been implemented to prevent, slow, or mitigate the damage (before the attack occurred).
Devise a plan to recover from this incident.
