Assignment Requirements

I attended wokshop # 7 on quantitative, during the session we shared our questionnares with the among other attendies as a result I recieved a brief feedback and I’m little bit concerned this is why I want to highlight directly to you. (Please advice on the following:
Pretains to my questionnaire, it was suggested that the relationship of two variables cannot be utilized within one question. Whereas, I have repeatedly used two variables within one question. For example;
a) Your productivity has significantly decreased due to stress
b) You feel your productivity would improve with suitable stress management
c) Your health has been affected by workplace stress
d) Stress causes many conflicts between co-workers
(these are the few I’m writing to you from my questionnare and still there are few more)

Would it be fair if I draft a new questionnare with one variable in each question to measure an average outcomes.
This time what if i use these variables ( job satisfaction, increase employee and organizationsal performiance including stress management machenasiam’s questions)
By the way i have not copied this questionnaire from any resarcher its newly build.
