Clinical Vignette 2: Gerado is two month old who was identified during newborn screening with sickle cell anemia. Gerardo has now developed acute chest syndrome in this case. He has been cared for in a comprehensive clinic in which he sees many different providers. This clinic serves as a medical home for his care.
