Details of task: Prepare a data collection instrument that could be used in the evaluation plan for the project BELOW (AT2. The instrument may be for quantitative and/or qualitative purposes. The task should have two parts (both included in the word count). These are: 1. the instrument you have developed, and 2. a 1-2 page discussion of the purpose of the instrument, the data it is intended to collect, and the considerations that have guided the way it has been designed. This discussion might consider the wording, format and order of the questions included, and other issues relevant to measurement quality. Please note that it is acceptable to integrate measures from other sources into your instrument, as well as developing your own.
Outer Western Sydney Food Project was a four year project that aimed to increase the proportion of residents in one socially disadvantaged local government Area who reported regularly consuming healthy foods. Funded primarily by the regional health service it involved a partnership between community health workers, local government, schools and business representatives.
Set out below are the goal, and a selection of the key objectives and strategies of the project.
Goal: Increase by 5% the proportion of families who report that both adults and children meet national guidelines for consuming vegetables, fruits and limited amounts of high energy snacks and drinks.
Target population: Parents of school aged children
Objectives:

Achieve 80% knowledge of guidelines for children and adults about consumption of fruits, vegetables, water and high energy snacks and drinks.
Reduce by 20% the proportion of parents who report that they have difficulty obtaining access to affordable, nutritious foods.
Increase to 60% the proportion of parents who report that they are confident to provide a meal for their child that meets the dietary guidelines.
Ensure the canteens in all primary and high schools meet nutrition guidelines.
Achieve 80% of sporting and leisure clubs and public facilities stock fresh foods, water and low fat products.
Strategies:
i. Establish a Food Policy Committee with Council to develop policy and liaise with transport providers and retailers concerning food access.
iv. Deliver evening seminars and distribute nutrition information and food preparation tips to parents through schools and through council newsletters.
v. Conduct workshops with school representatives, followed by information materials, monitoring and an accreditation system to increase the proportion of schools which meet healthy canteen guidelines
vii. Conduct workshops with sporting clubs and facilities representatives followed by information materials and an accreditation system to increase the proportion which meet healthy food guidelines
viii. Undertake cooking demonstrations at neighbourhood centres and community festivals and open days.
