Target population can be a city, a neighborhood or county. Describe the overall health status of this community. Identify the
main health issues. Selecta a priority Health Issue to be addressed. Justify your selection. Identify the data sources that
you will use
PAPER BASIC OUTLINE
? Introduction [This section must include a brief description of the the overall purpose of the project and provide a general
overview of the health issues and background information of the relationship to the population selected. This must be
supported by peer reviewed literature evidence.
o Health Issue: Review the literature relevant to the health issue. Discuss the specific health issue including major social,
cultural and behavioral practices /factors related to the health problem.
o Target Population: Discuss the targeted population in your project, and demonstrate how it is at risk for the identified
health issue. Make sure to include epidemiological data to support your selection of this population. Include the major
social, cultural and behavioral factors that are involved.

o Integration of Health Problem and Target Population: Integrate the theoretical concepts and constructs to the health
promoting or harmful behaviors in this population that are relevant to the health issue. In doing so you need to identify the
social, cultural and behavioral factors involved and explain how the risk and protective factors of the population contribute
to the problem/disparities.
