RE: STEPHEN MAYNARD MR.

• The above named presented to this facility with acute pain c/o 46.

• OE: T.T.P 46 with composite Pf that was still intact OB.

• Options of either Rct or Extraction discussed at length with attending pros and cons. Patient opted for extraction with an intention to have a 3 units PBM bridge done after healing period of 2-3/12.

• Rx done: Xla of 46, discharged on analgesics and antibiotics.

• To have a bridge done emphasized to avoid shifting and supraeruption.
