PRACTICUM PROPOSAL: Develop an initial written set of practicum objectives and activities (1-2 pages long) to be approved by the agency supervisor and the gerontology practicum coordinator. Submit the proposal via Canvas.

The practicum proposal must include: • A cover page with the following:

• Name, address, and phone number of the agency. • Supervisor’s name. • Starting and ending dates for the practicum. • Days and times you will be at practicum agency. • Signatures of student and supervisor (along with the date). • A proposal summary that includes: • List of learning objectives • Activities to fulfill these objectives.

This practicum is being done in a nursing home !
