This will have to include the following:

(brief overview of the injury/disease and what patient presents with and their limitation to what they can do and can not)
2. Body of evidence
(this I will provide from SOAP note and EVAL note, this includes OBJECTIVE section, on things done with patient, intervention and everything else

3. Conclusion
4. References

I will provide more info with soap note. But will have to verify writer first. This is a piece of cake paper because its basically like a copy paste from what happened in the paper but in terms of research format. So please spare me some BS with overpricing. If you have experience in health care field this is done in 1 hr.

The writing format does not necessarily be AMA but that is preferable. Basically, when you reference put a small number on top of reference page as long as what is stated has a reference page linked to the number ( as a writer you should know this 🙂
